Job
Summary:
Serve as the Agency's liaison to State Developmental Centers.
Provides consultation, advocacy, resource information and service coordination to the developmentally disabled community.
Primary Job Functions:
1.
Conducts strength-based assessments which communicate an understanding of the unique issues facing family members.
2.
Comprehensive assessment must be completed and submitted in a timely manner as required by law.
3.
Involve individual and family in all aspects of person-centered planning and support activities.
4.
Design, document, implement and monitor services and supports in a timely manner.
5.
Facilitate planning team progress, identifying outcomes, needed services and supports.
6.
Develop and maintain a network of vendors and community agencies.
7.
Provide relevant information and training about developmental disabilities, services and resources.
8.
Use effective communication and interpersonal skills, which consistently respect children and adults with developmental disabilities and their families.
9.
Participation on IDT (interdisciplinary team), including participation with IPP planning and transition planning.
10.
Annual face to face with individuals served on the residential unit, more often when there are issues that warrant follow up or requests by individual served, family, Developmental Center staff, Regional Project staff or for purposes related to court commitment.
11.
All Special Incident Reports must be called, or submitted by electronic written communication, into the DC Liaison by the Developmental Center Staff, (usually done by the DC social worker), these reports of incidents should be recorded in the T19 notes, as well as written notices filed in the persons served chart.
SIR's involving a death must be reported to DDS as well.
12.
To advocate for individuals served to reside in the least restrictive environment per regulations in the Lanterman Act, to assist with identifying needed services to continue to reside in the least restrictive environment, and advocate for provision of such services, even if this may be against conservators wishes, per the Lanterman Act.
13.
Effective verbal or written communication with individual served, staff and family on a minimum quarterly basis or as requested.
14.
Documentation of all interactions with individual served, family, staff and those involved with the individual served in T19 notes.
15.
Ability to provide information on individuals in the developmental center to DDS, Social Security, Medi-Cal, the courts, etc when requested by the individual or conservator.
16.
To monitor court re-commitment dates, including 6500 and in re Hop (court commitments) and to follow up with attorney to ensure timely compliance with these and other legal matters.
17.
To direct to the individual's chart any information or pertinent legal documents following review of information.
Skill Requirements:
Knowledge of developmental disabilities and dual diagnosis Knowledge of DDS regulations regarding DC closures Knowledge of evidence-based practice interventions relating to developmental disabilities and dual diagnosis.
Knowledge of WIC 4418.

Qualifications:
Master's or bachelor's degree in human services field or a related area, with 3 years of applicable experience.
Placement experience.
Valid California driver's license and reliable, insured vehicle or equivalent means of transportation.
Able to spend 30% - 50% of work time traveling and away from home.
Physical Requirements:
Adequate manual dexterity and coordination for operation of a computer and other standard office equipment is required for this position.
The ability to sit at a workstation for long periods of time, the ability to read, write and comprehend large amounts of written material such as reports and regulations, use the telephone and computer and communicate via electronic mail are all required on a daily basis.
The ability to travel to another work-site location is absolutely required on a daily basis.
The ability to stand, sit or walk for moderate distances on uneven terrain is also required on a daily basis.
While performing the duties of this job, the employee is regularly required to use hands to finger, handle, feel or operate objects, tools, or control and reach with hands and arms.
The employee frequently is required to stand, sit, walk, push, pull, kneel, stoop, bend, and squat.
The employee must frequently lift and/or move up to 10 lbs and infrequently up to 20 lbs.
Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and the ability to adjust focus.
Each of these physical tasks is an essential function of the position.
